Nomination Form
The Ten Awards Official Nomination Form, describing the nominee’s direct contribution to their industry and beyond, should be completed in full and submitted to the Executive Council.

Factors to be considered by the Judges include: Nominee’s impact on innovation; Nominee’s impact on industry and globally; and Nominee’s leadership and standing within peer group and outside of peer group. Read Judging Criteria.
